Step One

An initial referral conversation will take place for any further information needed.
Step Three
If the referral is accepted. Our comprehensive referral form will be issued for completion.
Step Four
Once a completed form is received. The matching process starts, and a home visit is arranged.
Step Five
During the home visit we will start our assessment on what support is to be provided, take background history and what the aspirations are for the future.
Step Six
The bespoke package is created, and practitioner is matched. A start date will be issued once contracts are signed and payment agreed.

Our vision is centered around being flexible, patient and creative. To enable us to create a bespoke package for each referral. Taking into account the needs, aspirations and overall outcomes of each individual.
